2 Graft CARE
Extraction with Expert; Manual or Motorized extraction with DrT. The Manual FUE is the first FUE technique which is done completely manually. In time, doctors have used various names for this method. But it is mainly known as Manual Punch. Both in manual and micromotor extractions, the crucial point is to harvest the hair follicles as a whole without damaging.
As the name suggest; it is managed by hand and it contains inside a cylindrical blade with diameter of 1mm. Follicular units are removed manually from the skin by rotating the pen shaped tool by hand. In Manual FUE, as well as in micro motorized FUE, the length of the follicular units the punch depth is determined manually accordingly and the follicular units are collected by hand one by one. It is possible to extract 1500-2000 follicular units per day. It may be considered to remain less scar on the donor area after extraction when it is compared with micro motorized FUE. But so far there is no medical evidence to prove this.

The most important question is who will perform the FUE Hair Transplant ?
A manual punch FUE operation should be performed by the doctor himself who has gained many years of experience with the FUE method. Before the treatment the structure of the hair follicle and the skin must be analyzed and a detailed treatment plan should be established. The Manual Punch FUE technique requires a dermatological analysis by extracting a few hair follicle samples. After the determination of the depth of the units, the treatment can begin. A lot of aspect needs to considered in order to select the right equipment, such as hair thickness, fat level or the structure of the hair unit. These variables have direct affect on results, therefore it is essential that the analysis before the treatment is done in great detail by the attending Surgeon.

The most important point in FUE hair transplantation is the long term storing of the extracted grafts in their ambient in the body just like an organ. To such end many researches have been conducted for long years; in operations lasting more than 4-5 hours the grafts start to die as a result of the changes of the ambient inside and outside of the cells; the graft loss rate is very high in operations lasting longer than 4 hours as the intracellular and extracellular balances of the grafts which are kept in normal saline or solutions such ringer lactate deteriorate.
Towards eliminating such a problem, it was determined that keeping the grafts in a special mixture of 2 special solutions named as Hypothermosol-FRS and ATP caused the grafts to stay 99% alive even for 24 hours and that high rate of success was reached in hair transplantations. In the scientific research signed by Dr. J. Matthew published in HAIR TRANSPLANT FORUM a scientific occupational periodical of ISHRS, the International Hair Transplant Union, 100% graft liveliness was specified in graft storing solutions formed by Hypothermosol-FRS and ATP in hair transplantation operations lasting for 6-12 hours and it was scientifically proved that the graft loss occurred around 27-44% when normal saline or solutions such as Ringer Lactate were used for graft storing purposes, additionally it was reported in the same periodical that adding serums such as PRP to storing solutions did not have any effect and that PRP was meaningless from the aspect of graft liveliness as the PRP could only activate when it is injected in the tissue and the Platelet cells could not activate outside of the body. 4) Donor CARE
PRP is the most frequently used medical treatment in the field of rapid amelioration of the donor zone and transplantation zone. PRP has been started to be used for the purpose of healing wounds or ameliorating some problems in field of Medicine for long years. PRP is in essence a serum which is very successful in the repairing of the tissue. We as well apply it as a standard especially for the purpose of rapid closing of the micro holes in the graft obtaining locations in the donor zone and to both the donor zone and the transplantation zone to the end of reducing the marks.
5) After CARE
ATP spray;In medical science the ATP is the energy molecule of the cells and it gives life to the cell. The ATP is started to be applied to the transplantation zone 4 hours after the spraying operation and applied to the zone every hour for a period of 5-7 days. ATP is as well the energy supply of the hair root cells as it is for entire cells. Newly transplanted hair must absolutely be started to be fed through a micro vessel in the zone where they are located, and usually a period of 72 hours is required for the formation of such a micro ambient and such vessels. In such period the grafts need humidity and energy in order for them to live. When it is considered that it is not fed through any blood vessel for the first 72 hours, the ATP is the only feeding source of the transplanted roots. Averagely 30-40% of the transplanted roots cannot stand such a period of 72 hours and retreat. This means that the transplanted roots withdraw the hair roots on them within the period of 2-4 weeks in order to save themselves and re-enter into the phase of hair production which shall last for 3-6 hours.
However 30-40% of the transplanted roots cannot produce new roots as they cannot be fed in the first 72 hours and the largest complication seen in FUE emerges that is to say some transplantation zones which have been observed to be thick in the course of transplantation is now thin and far from an aesthetical look at the end of a period of 1 year. ATP with its such feeding feature keeps the whole of the transplanted hair strong, in so much that while the rate of directly growing hair without being shed was around 10% in the first 1 month period, such a rate could be increased up to 30%.

Follicular units are removed manually from the skin by turning. In manual FUE, as well as in micro motorized FUE, according to the length of the follicular units the punch depth is determined manually and the follicular units are collected by hand one by one. It is possibile to extract 1500-2000 follicular units. It may be considered to remain less scar on the donor area after extraction when it is compared with micro motorized FUE. But so far there is no medical evidence to prove this.
The possibility of damaging to the follicular units is low because of the minimize speed rotation of the punchs. But at this point there is no any medical work or evidence.
Micro motor is a tool also called as FUE engine. As the name suggests this tool which contains a small motor inside is an electrical device with it’s special ends. Micro motor tool has different sizes ends, the diameters of these ends can range between 0,6-0,9 mm depending on the patient’s skin and hair.
The ends of the micro motor penetrates into skin by spinning during the extraction of follicular units and allows the extraction of follicular units easily. Punch depth is determined manually according to the length of the follicular unit.
The sessions generally takes less time. Because with micro motor tool the procedure is done more quickly.ıt is possible to extract 3000-3500 grafts follicular units a day and they are collected manually. Because of the importance of rotational speed of punch it may be considered to be damaging to the donor area. But this is not true when is realized by an expert.
